Please bring copies of the results of any bloodwork or imaging studies you have had that pertain to the reason for your visit. Please bring a list of any medications you take (including herbal supplements, vitamins, etc.).

Please wear or bring comfortable, loose-fitting clothes.

24-hour notice is required for cancellations; otherwise, the full appointment fee will be charged. As a courtesy, we will contact you to confirm your appointment in advance, and Dr. Blyskal makes every effort to see her patients on time.

Payment is required at the time of the visit; you will be provided with a receipt that you may submit to your insurance company if you are eligible for reimbursement.

Please note that Dr. Blyskal sees Medicare patients independently of Medicare coverage: patients with Medicare would thus contract privately with Dr. Blyskal and be unable to submit claims to Medicare.

Please note that if you wish to send e-mail communication to and receive e-mail responses from Dr. Blyskal, you should be aware that e-mail is not a secure medium for potentially sensitive personal health information. Although Dr. Blyskal will take steps to keep your communications confidential and secure, the confidentiality of e-mail communications cannot be assured or guaranteed. You also acknowledge and understand that e-mail is not a good medium for urgent or time-sensitive communication. In the event a communication is time-sensitive, you must communicate with your physician by telephone or in person. You acknowledge and understand that, at the discretion of Dr. Blyskal, your e-mail may become part of your permanent medical record.
